1. Mission and Main Focus of Work

TB Mukt Vahini is a Bihar – India based network of TB survivors committed to supporting people with TB, raising community awareness, and empowering TB-affected individuals and their families. Our mission is to reduce TB-related stigma, ensure access to quality care, and advocate for effective policies that support the TB community. We emphasize survivor-led initiatives that engage communities in overcoming challenges in TB care, treatment adherence, and reducing socio-economic barriers to accessing healthcare.
Our primary focus includes:
• Empowering TB survivors to lead community mobilization efforts.
• Running anti-stigma campaigns to foster inclusive attitudes.
• Facilitating nutritional and psychosocial support programs for people with TB, including the Nikshay Mitra Food Basket program in collaboration with local partners.
•Advocating for policy-level interventions to enhance TB care infrastructure in Bihar.

2. Why We Are Interested in Tuberculosis

TB Mukt Vahini's interest in tuberculosis stems from the lived experiences of our network members, who are TB survivors themselves. These experiences highlight the urgent need for community-driven approaches to address:
• Persistent stigma and misinformation around TB.
• Challenges in treatment adherence due to socio-economic vulnerabilities.
• The systemic gaps in healthcare delivery for vulnerable populations, especially in resource-constrained settings like Bihar.
We believe that survivors’ voices and leadership can drive impactful solutions for TB control, helping to address these challenges in a culturally sensitive and sustainable manner.

TB Mukt Vahini actively supports India’s National Tuberculosis Elimination Program (NTEP) by aligning its efforts with the government’s goal of eliminating TB by 2025. Our contributions are focused on strengthening community engagement, addressing social determinants of health, and complementing healthcare service delivery, particularly in the state of Bihar.

1. Supporting Treatment Adherence and Nutritional Support
• TB Mukt Vahini has implemented the Nikshay Mitra Food Basket Program, ensuring that TB patients receive essential nutrition, which is a critical component of their treatment and recovery.
• By working closely with local stakeholders, including Lions International and community-based organizations, we have created sustainable models for patient support, in line with NTEP’s emphasis on patient-centric care.

2. Promoting Early Diagnosis and Timely Treatment
• Our community awareness campaigns emphasize the importance of early TB diagnosis and adherence to treatment regimens. These campaigns help increase demand for NTEP services like free diagnostic tools (GeneXpert, sputum microscopy) and medication, reducing delays in seeking care.

3. Reducing Stigma and Discrimination
• TB Mukt Vahini conducts anti-stigma drives in communities, workplaces, and educational institutions to combat misconceptions about TB. By sharing stories of TB survivors and fostering open discussions, we address one of the key barriers to TB care: stigma and discrimination.
4. Building Community Leadership

• We train TB survivors as community champions, equipping them to act as peer educators, treatment supporters, and advocates for the NTEP. Their involvement ensures that services reach marginalized and vulnerable populations in remote areas.

5. Advocacy and Policy Support
• TB Mukt Vahini works with local health authorities and development partners to identify gaps in service delivery and propose solutions. This includes participating in consultations, such as program-wise discussions led by the National Health Mission (NHM), to review progress and develop action plans for achieving NTEP targets.
• We also advocate for enhanced resource allocation, improved diagnostic infrastructure, and policies to address social determinants like malnutrition and poverty.

6. Expanding Awareness on New TB Tools and Strategies
• As a survivor-led network, we actively engage with global and national stakeholders to promote awareness about new diagnostics, shorter drug regimens, and innovations in TB care. This helps in aligning community perspectives with advancements in TB control efforts.

7. Community-Led Monitoring and Feedback
• Through our grassroots presence, we provide valuable insights and feedback to policymakers and healthcare providers about the challenges faced by TB patients and survivors, ensuring that the NTEP remains responsive to community needs.

TB Care Delivery:

Current Contributions:

Advocacy and Awareness Campaigns: Since 2017, TB Mukt Vahini has conducted over 50 anti-stigma drives in Bihar, India, raising awareness about TB symptoms, treatment, and the availability of free diagnostic and treatment services under the National TB Elimination Program (NTEP).
Community Empowerment: Engaging TB survivors as community leaders to support patient adherence and advocate for quality TB care in resource-limited areas.
International Engagement: Participation in The Union World Conference on Lung Health (Bali, Indonesia, November 2024) with a focus on survivor-led strategies for TB care delivery.
Future Plans:

Expand our survivor-led advocacy model to other high TB-burden countries through collaborations with global partners such as the Stop TB Partnership.
Develop multi-lingual communication toolkits for social mobilization campaigns globally, particularly in underserved areas.

Drug-Resistant TB:
Current Contributions:

Supporting DR-TB patients in Bihar by providing nutritional and psychosocial support, which are critical for treatment adherence.
Support in Pre treatment evaluation of diagnosed DR TB person
Advocacy for access to shorter, patient-friendly regimens like BPaL (Bedaquiline, Pretomanid, and Linezolid) for DR-TB patients through community engagement events.
Future Plans:

Collaborate with global stakeholders to introduce community-centric DR-TB care models in high-burden settings.
Conduct workshops on the adoption of digital adherence technologies for DR-TB patients, ensuring proper treatment monitoring and reducing loss to follow-up.


TB-HIV:
Current Contributions:

Partnering with local health authorities to raise awareness about HIV-TB co-infection, emphasizing the importance of integrated care, testing, and early detection.
Promoting adherence to ART (antiretroviral therapy) alongside TB treatment for co-infected individuals in rural areas of Bihar.
Future Plans:

Advocate for better integration of TB and HIV programs at the community level globally, ensuring seamless service delivery for co-infected patients.
Engage with international conferences and platforms to share best practices and insights from India’s TB-HIV programs.

Commitment to Transparency and Cooperation

TB Mukt Vahini is fully committed to disclosing any relevant interests should they arise in the future. We understand and respect that the Executive Secretary of the Stop TB Partnership may review declared interests to determine whether they pose an appreciable real, apparent, or potential conflict of interest concerning specific projects or activities.

If it is determined that any declared interest disallows participation in a specific project or activity, we acknowledge the right to appeal to the Coordinating Board for reconsideration.
