1) Bagmati Welfare Society Nepal (BWSN) is a non-governmental organization dedicated to improving the well-being and quality of life for marginalized and disadvantaged communities in Nepal. Its mission is to enhance socio-economic conditions and promote human rights, empowering underprivileged populations through access to basic services such as healthcare, education, and economic opportunities, while advocating for social justice and equality. BWSN focuses on several key areas, including health and sanitation, where they implement programs to improve public health, maternal and child care, and sanitation in rural regions. In education, the organization works to ensure access to quality learning, particularly for disadvantaged children, including girls, Dalits, and ethnic minorities. Economic empowerment is another core area, with BWSN offering training and support for income-generating activities to uplift low-income households, particularly targeting women and youth. Additionally, BWSN advocates for human rights and social justice, raising awareness and engaging in policy advocacy for marginalized groups to ensure social inclusion and equality. BWSN implements the interventions to catch up with the target and objective of NSP. The National TB prevalence survey 2018-19, currently over 117 000 people are living with TB disease in Nepal. Likewise, 69 000 peoples developed TB. To confront the burden of TB, the country has developed a national TB strategic plan (TB-NSP) that covers the period 2021/2022 and 2025/26. This TB-NSP has ambitious TB impact targets which include decreasing TB incidence and mortality from 238 to 181 per 100,000 and 58 to 23 per 100,000 respectively, between 2021/22, year one of the TB-NSP and 2025/26. Additionality, TB presumptive case visiting OPD, Family members of PBC and less than 15 years TB cases, Family members of DR TB cases, SAM, MAM and ARI cases and Presumptive DR TB cases. Poor nutrition, migrate population and inadequate living conditions all contribute to the spread of TB and its impact upon the community.
Aditionally, 1 Awareness Campaigns: Conduct public education campaigns to increase awareness about TB symptoms, transmission, and the importance of early diagnosis and treatment. 2 Screening and Diagnosis: Provide resources for widespread screening and rapid diagnostic testing to identify cases early and Treatment. 3 Treatment Support: Advocacy for access to treatment center, including directly observed therapy (DOT), to ensure adherence and successful outcomes. 4 Training Healthcare Workers: Training programs for healthcare professionals on TB management, diagnostics, and the latest treatment protocols. 5 Data Collection and Research: Conducting research and collecting data to inform national strategies and identify trends in TB prevalence and treatment outcomes. 6 Collaboration: Partnering with governmental and non-governmental organizations to align efforts with national priorities. 7 Policy Advocacy: Advocating for policies and funding that prioritize TB control and ensure resources are allocated effectively.

TB Care Delivery: Global Fund is created fund by the initiation of developed countries to fight against the Tuberculosis, Malaria and HIV and AIDS. Mechanism of the funding of the global fund has been a very much different from other funding. The mechanism of the Global fund can be described as below chart:
Global fund was created in year 2000 to achieve the Millennium Goal to stop TB and HIV/AIDS. The Global Fund has been funding to different countries since 2000. Nepal has received the grant of global fund on 2002 as the 2nd round and some the continuation of the intervention as the R7 & R10 and now it is funded as NFM (New Funding Model) Grant of global fund in Nepal is being managed by Save the Children (SCI) under the technical leadership of National Tuberculosis Control Center (NTCC). Under the new funding model, SCI has provincial level sub recipients (SRs). Both of Madhesh and Koshi Province, Bagmati Welfare Society Nepal (BWSN) is implementing the project in close coordination with provincial government and local government. The project activities are designed and approved by Global Fund based on the National Strategic Plan (NSP) (2021-2026) which is owned by the government of Nepal. As a result, BWSN implements the interventions to catch up with the target and objective of NSP. The indicators are basically focused to the following activities. • TB case detection in hard-to-reach population by establishing sputum courier system to microscopic centers. • Mandatory contact tracing to family members of PBC and all childhood TB cases • TB screening in malnourished children in Outreach clinics/ Health facilities • TB screening in malnourished children in major hospitals of districts • Screening and testing of all presumptive DR TB • Screening of Family members of index DR TB Cases • Establish incentive system to the private sector that are substantially (physician) handling TB cases by establishing pay for performance mechanism • Implementation of FAST strategy at major hospitals. • ACF at Migrants Prison Population / risk and vulnerable population (Pharmacy referral, hotspot screening) • TB Preventive Therapy (TBPT)
