Declaration

Declaration of interests:
TB detection is through passive case finding and only 43% of the estimated incident cases are identified
annually. Tuberculosis in Somalia affects mainly the young and most economically productive age-group.
The highest proportion (56%) of sputum smear positive cases notified in 2013 were from the 15-34 year
age-group. The other age-groups 35-44, 45-54, 55-64 and >65 years contributing 16%, 10%, 8% and 6%
respectively. Tuberculosis is rarely being reported in the under 15 years children which contributed only
4.6% of 2013 sputum smear positive cases. The women contributed 37% of the sputum smear positive TB
cases notified in 2013. Treatment success of sputum smear positive cases has been over 85% for several
years now. However, MDR-TB prevalence is estimated at 5.6% for new TB cases and 40.8% for the
retreatment cases.
The organization of the TB programme is run as a vertical programme. The government of Somalia and all
TB stakeholders have resolved to integrate TB into the primary health care. This is an effort to increase the
case detection rate from the current 43% of all the estimated incident cases annually. The big gap in the
detection of Tb is mainly due to inadequate access of TB services to the population. The strategy is to
improve access by integrating the TB services into primary health care. Advocacy, communication and
social mobilization (ACSM) has not been prioritized previously. Other aspects highlighted in this strategy
includes the strengthening TB management in children, expanding MDR-TB management, initiation of TB
in prisons, introduction of TB school programme, start of outreach programmes to reach remote areas and
meet the needs of nomads and capacity build the national authorities for better management of TB control.
It is proposed to explore the option of using Mhealth (using mobile telephone) for training and awareness
raising for the public. Integration is also expected to minimize the loss of patients during referral from
general health services to TB
