1- At PHC Global, we focus on improving population well-being and health outcomes through collaborative efforts with public service delivery institutions, academia, and development partners. The organization’s mission is to work with enabling collaborators and clients to facilitate the well-being of people, communities, and the environment through a systems approach, evidence-based research, and the adoption of innovative technological interventions. We envision a world where every individual is safe and well with access to effective services and quality healthcare resources to address the most pervasive challenges in the health ecosystem. We aim to improve the quality of health services using client-centered designs at health facilities. We use innovations to adapt and design need-based training platforms to enhance the health worker's capacity to address the challenges of infectious diseases, especially in resource-constraint settings. Most of our work focuses on primary prevention of infectious diseases through immunization, and improving the effective coverage of maternal, child and adolescent health interventions. In parallel, we focus on quality of care with public sector health systems, to improve compliance to treatments such as antimicrobials to reduce the burden of drug resistance in Pakistan.

2- The Annual TB Report 2019 Pakistan ranks 5th among 30 high-burden countries for TB and 4th for Drug Resistant TB (DR-TB) . The incidence of MDR-TB is 1605 cases annually. TB incidence shows an upward trend while the TB mortality rate declined over the past decade. However, TB case notification remained at the lowest in Baluchistan (10%). According to study settings comparable to Baluchistan province, 29% of reported TB cases are defaulters after primary treatment but according to the Provincial TB Control Program (PTP), Baluchistan, between 2014-18 only 2% (range 2% to 3%) of the patients were categorized as retreatment cases. Our proposed program aligns with the provincial strategic plan for TB control in Balochistan to reduce the incidence of DR-TB. The strategic areas that are the focus of the provincial health department and our proposed program are given below:
•Increase the notification for DR-TB and maintain the treatment success rate by expanding case finding and strengthening services.
•Increase enrollment of DR-TB in the TB Program and increase their treatment success rate by improving services at PMDT sites, and enhancing pharmacological, psychological, and social support.
•Increase community awareness and sensitization, strengthen the capacity of HCPs and TB programs increasing monitoring and evaluation support.
•Generating evidence through operational research for policy change.

Contribution

Please tell us how your organization will contribute to the Global Plan to Stop TB by briefly describing its involvement in any of the areas of work listed below:

Research:
Our proposed intervention focuses on improving the quality of care in public sector in Balochistan, Pakistan for disease prevention, correct diagnosis, and providing appropriate treatment to reduce the burden of drug-resistant Tuberculosis (DR-TB). PHC Global is partnering with Fatima Jinnah Institute of Chest Diseases (FJICD), which is one of the only two Programmatic Management of Drug Resistant TB (PMDT) sites in the Balochistan province of Pakistan, to implement the intervention. The PMDT site serves as an ambulatory primary care center with linkage to the inpatient services at FJICD. The intervention has a two pronged approach i) engage patients diagnosed with DR-TB to increase their compliance with the management and treatment guidelines and address proximal barriers to accessing timely and quality healthcare, and ii) train PMDT staff on a) improved psycho emotional counseling, b) complete history taking, c) correct diagnosis and d) adequate treatment plan for patients. The intervention will also train how both patients and providers can play their part in stopping the spread of DR TB, and collaborations are built within the healthcare system.

The proposed program intends to reduce the spread of the disease by improving at the systems level treatment compliance of diagnosed patients. The intervention is geared to strengthen the healthcare system as it contains preventive, curative and promotive components which will reach the community through the enrolled patients serving as peer advocates. The proposed program will address these barriers through (a) increased HCPs’ capacity for case detection and TB notification through systematic screening, (b) accessible quality care and improved preventive measures, (c) peer advocacy, (d) increasing awareness through the eLRF, especially in remote areas of Balochistan province (e) patient education and communication (f) family counseling, (g) skill and knowledge-based training of HCPs to influence positive behaviors of patients and community members, and (h) delivery of patient-centered psychosocial intervention for increasing HRQoL and motivation to comply.
