World TB Day - 24 March 2001

                                                             Briefing on STOP TB Global Events

as well as new information on any other events taking place around the world]


The theme of World TB Day 2001 (DOTS: TB Cure for All) emphasises the need to increase access to effective treatment for people with TB. The number of people with TB increased 6% between 1997 and 1999, yet less than 1 in 4 people with TB is treated with DOTS, and DOTS expansion is much slower than that required to reach the global targets by the year 2005.

This year sees a departure from the usual format for World TB day events. To draw attention to the global nature of the epidemic, and to highlight the importance of the Stop TB partnership in addressing the epidemic, World TB Day will be commemorated by a series of coordinated and complementary events in both high burden and low prevalence countries, organised by different Stop TB partners. Each event will emphasise a specific feature of the World TB theme, with the overall aim of promoting awareness, and increasing access to care.

Events that succeed in gaining the attention of the global media clearly have a significant part to play in the World TB Day campaign. The Stop TB Initiative will monitor press coverage, and compile press clippings for ‘TB Times’, to be published immediately after World TB Day.

However, the ‘backbone’ of the campaign is, as usual, the myriad of events taking place at national and local levels. As in previous years, Stop TB has asked those organising World TB Day events to report on their activities, for publication in ‘World TB Day Highlights’. For the first time, this publication will not only highlight events, but will focus on tangible achievements in increasing DOTS coverage that directly resulted from World TB Day activities.

High quality advocacy materials have been a regular feature of World TB Day over the last few years. For the first time, these materials have been commissioned from a Stop TB partner, the International Union Against TB and Lung Disease, with partners sharing responsibility for distribution. Materials have been distributed well in advance of World TB day, to enable countries to translate and adapt materials for their own use.

South Africa: Friday 16th March

Archbishop Desmond Tutu will launch World TB day with a press conference in Uitsig, South Africa, and announce the publication of the Stop TB report on A Human Rights Approach to Tuberculosis. The event is organised by IUATLD and the University of Stellenbosch. The AFRO Regional Director has been invited to attend.

Washington: Wednesday 21st March

The American Lung Association, the American Thoracic Society, Results International, and the Stop TB Partnership Secretariat is organising a joint event to launch the ‘Omnibus Tuberculosis Control Act of 2001’ and the Global TB Drug Facility. Speakers include Dr Gro Harlem Brundtland (on video), and Dr JW Lee. The Omnibus Tuberculosis Control Act of 2001 developed by the NCET calls for a massive increase in US funding for domestic and international TB control activities. The GDF complements this call, by demonstrating the need for rapid DOTS expansion, and providing a mechanism to increase access to high quality TB drugs. Documents to be launched at this event include the GDF prospectus, and the Global Report on TB Control 2001.

In Darmstadt haben wir im letzten Sommer erlebt, dass eine Studentin aus Moskau – statt Lebensbedingungen von Frauen in Ost und West zu untersuchen – unerwartet mit offener Tuberkulose in die Isolierstation der Städtischen Kliniken eingeliefert wurde. Aus den Medien erfahren wir, dass das kein Einzelfall war, sondern dass seit Mitte der 90er Jahre TB wieder auf dem Vormarsch ist. Weltweit erkranken jedes Jahr 8 Millionen Menschen an TB und nur 16 % der Patienten werden therapiert. Armut, Verelendung, Kriege, Migration und der Zusammenbruch öffentlicher Gesundheitssysteme, aber auch der wachsende internationale Reiseverkehr fördern die Ausbreitung des Erregers. In den Entwicklungsländern gefährdet die TB die wirtschaftlich produktivste Altersgruppe: 80% der Kranken sind zwischen 15 und 49 Jahre alt.

Die Weltgesundheitsorganisation (WHO) rief im Oktober 2000 auf ihrem Gesundheitsforum gegen Armutskrankheiten in Winterthur zur „Massiven Anstrengung" aller auf, weil nur eine unverzügliche Bekämpfung, wie auf dem Gipfel in Okinawa von allen beteiligten Staatschefs verabredet, das schmale Zeitfenster zur Bekämpfung der TB mit preiswerten Medikamenten ermöglicht. Andernfalls besteht die grosse Gefahr, dass sich resistente Tuberkel-Stämme entwickeln, die die Kosten der Behandlung vervielfachen und in armen Ländern undurchführbar machen würden.

Aufgrund der Beiträge von 1999 schätzt die WHO, dass die wichtigen Industrienationen entsprechend ihrer Anteile an der International Developing Agency (IDA) ihre Beiträge wie folgt erhöhen müssten, um die weltweit notwendigen 2 Milliarden US Dollar aufzubringen:

23 March, Moscow, Russia, MSF held a "Contagious Concert", in which young musicians perform works by composers who suffered from tuberculosis — including Chopin, Liszt, Shostakovitch, and Stravinsky.


USA, CDC

Dear Colleagues:

As you know, World TB Day is March 24th and is rapidly approaching. On this day in 1882, Dr. Robert Koch announced the discovery of the TB bacillus. Observance of this day is intended to raise awareness about the devastating health and economic consequences of TB, its impact on developing countries, and its continued threat to global health. The Centers for Disease Control and Prevention (CDC) encourages you to develop specific state or local activities that will raise awareness about TB and take advantage of the press interest that World TB Day will stimulate. CDC's theme for World TB Day this year is "TB Elimination: Now Is the Time."

CDC is also planning a number of activities around World TB Day. A brief summary of these activities follows:

CDC World TB Day Observance. The Division of TB Elimination (DTBE) and the National Center for HIV, STD, and TB Prevention (NCHSTP) Office of Communications will sponsor an event at the CDC Global Health Odyssey on March 23. The Global Health Odyssey, CDC's museum, is a small exhibit area with an attached theater. An exhibit displaying vintage TB posters from around the world will be featured. This event will also involve guest speakers from 1) DTBE, 2) the NCHSTP Director's Office, 3) the CDC Director's office, 4) the American Lung Association (ALA), 5) the World Health Organization (WHO), and 6) a member of CDC's Advisory Council on TB who will share her story as a person who had TB. Internet. CDC will post information regarding World TB Day on the DTBE Web site the week of March 19. DTBE's Web site can be accessed at http://www.cdc.gov/nchstp/tb/ There will be general information about World TB Day, as well as links to pdf and html versions of the "Now Is the Time" fact sheet and trends document (from which text and graphics can be used), and two MMWR articles.

In addition, the site will feature an interactive map of World TB Day activities around the United States. Last year on World TB Day, a number of U.S. localities were very successful in attracting significant press attention to local as well as national and global TB problems. We still want to hear about this year's events. As soon as your event is planned, please send an e-mail summarizing the event to Mr. John Seggerson at jjs1@cdc.gov. We will post these activities as they come in on the DTBE World TB Day Web site.

DTBE will also link to other World TB Day sites at partner organizations, such as ALA, WHO, the Stop TB initiative, and the International Union Against TB and Lung Disease (IUATLD).

World TB Day Internet Spotlight. CDC will feature World TB Day as a "Spotlight" item on its homepage the week of March 24. There will be a link to the DTBE site for further information.

Media Kits for MMWR Articles. There will be two TB-related articles in the March 23 Morbidity and Mortality Weekly Report (MMWR). The related MMWR press releases will be embargoed until Thursday, March 22, at 4:00 PM. The NCHSTP Office of Communications is developing a media strategy and press kit around the MMWR articles. This press kit will be mailed to partners as soon as it is ready.

Accurate TB surveillance is essential in evaluating current TB control efforts, planning new strategies for TB elimination, and assisting policy makers in critical decisions. Provisional surveillance data for 2000 - submitted by the reporting areas to CDC - indicate that a total of 16,372 TB cases were reported from the 50 states and the District of Columbia, a decrease of 7% from 1999 and 39% from 1992, the height of the TB resurgence in the United States. The 2000 case rate of 5.8 per 100,000 population is 45% lower than the rate was in 1992 (10.5). The proportion of TB cases among foreign-born persons increased from 27% in 1992 to 46% in 2000. Once we finalize these data with the various reporting areas, we will provide a summary report in an upcoming issue of the Morbidity and Mortality Weekly Report.

We hope the provisional data, as well as the messages and resources enclosed in this packet, help you communicate to local media and organizations with which you work. These vital messages to communicate include both the real possibility of eliminating TB in the United States, and the consequences of not taking decisive action to eliminate this disease.

ACTIE ZONTA NEDERLAND VOOR TUBERCULOSEBESTRIJDING

Elke minuut sterven er 3 mensen aan de gevolgen van Tuberculose. Tot die dodelijke slachtoffers behoren jaarlijks 900.000 vrouwen, veelal moeders met jonge kinderen. Ieder uur komen er ergens ter wereld 900 tuberculosepatiënten bij. Besmetting met tuberculose ontstaat simpelweg door hoesten, maar hoeft niet in alle gevallen tot ziekte te leiden. Adequate genezingsprogramma’s bieden momenteel toegang aan 1 op de 5 patiënten. De Wereldgezondsheidsorganisatie (WHO) voert campagne om genezing binnen ieders bereik te brengen. Speerpunt is 24 maart, Wereld Tuberculose Dag. Dit jaar met als motto:’GENEZING VOOR IEDEREEN’. Kennis en ervaring om dat doel te bereiken zijn bij de internationale bestrijders voldoende aanwezig. Geld vormt het grootste struikelblok, terwijl genezing van een tuberculosepatiënt gemiddeld slechts ƒ120,-- kost.

De internationale netwerkorganisatie Zonta, vergelijkbaar met organisaties als Rotary en Lyons, plaatste tuberculosebestrijding hoog op de agenda en kreeg daarbij support van minister Els Borst. In Nederland haakte het Zontabestuur hier op in en deed samen met Johan Stekelenburg (voorzitter van het Nederlands Tuberculose Fonds) een oproep aan alle Nederlandse Zontaclubs. Vele daarvan zullen de tuberculosebestrijding de komende maanden onder de aandacht brengen en met ludieke acties geld inzamelen.

Het afgelopen decennium zijn buiten Nederland meer dan een miljoen tuberculosepatiënten genezen, voor een groot deel met financiering door de 80.000 donateurs van het Nederlands Tuberculose Fonds. Voor de komende jaren is het streven: toegang tot genezingsprogramma’s voor 70% van alle tuberculosepatiënten.

Iin Nederland worden jaarlijks 1.500 tuberculosepatiënten geregistreerd. Een aantal dat, met gemiddeld 4 per dag, positief afsteekt bij dat van 3 dodelijke slachtoffers per minuut elders in de wereld. Het genezingspercentage is vrijwel 100% en vormt een schril contrast met de situatie in ontwikkelingslanden. Toch blijft ook in ons land voorzichtigheid geboden. Daarom wordt bij elke melding een contactonderzoek gehouden, dat zeer omvangrijk kan zijn. Bijvoorbeeld onder bezoekers van discotheken, voetbalverenigingen of andere drukbezochte gelegen-heden. Om er voor te zorgen dat de ziekte in Nederland beheersbaar blijft zijn dagelijks gespecialiseerde artsen en verpleegkundigen actief. Voor de relatief veilige Nederlandse situatie schuilt de bedreiging in de toename van het aantal gevallen van resistente tuberculose. Dit risico groeit naarmate de bestrijding elders in de wereld minder effectief verloopt. Door de in Nederland aanwezige kennis en ervaring juist daar in te zetten wordt veel onnodig leed voorkomen en waarborgt men indirect de Nederlandse status quo.

Zonta Nederland bijt het spits af en ontvangt op 22 maart een Zonta-promotie-Smartcar die enkele maanden door Nederland toert voor het goede doel. Op zaterdag 24 maart, Wereld Tuberculose Dag organiseert Zonta-club Den Haag II een programma waaraan diverse bekende Nederlanders deelnemen, waaronder Johan Stekelenburg.

Ethiopia Event

The TB & Leprosy Control Programme of Ethiopia has the pleasure to inform you that World TB day will be commemorated on Saturday morningwith an attractive Radio and TV covered programme in the Ambassador Theatre in Addis Ababa. The celebration will be preceded by a press conference on Friday afternoon. To stimulate this commemoration the AHRI Institutes organises an "open house" with presentations and discussion on Thursday afternoon. Ten thousand leaflets on TB and the theme of the Day have been printed in Amharic. 5000 TB-T-shirts and paper caps have been produced and will be distributed. Banners are alreay put up in various places in town. The attachments shows a photograph of the organzing teams (jpg 44kb) and the banner (jpg 50kb) at the entrance of the Ministry of Health. The Video tape for the Day has now been received and will be presented to ETV for broadcasting.

India Event

World Tb Day will be celebrated with the Eminent Pulmonogist of the region under the Breathe Trust programme for Poor TB Patients of the region:

This will be the Biggest Free Diagnostic and Treatment Camp in the region.
